Connected Care – Community Health Centre at Market Mall

Access to the facility will be available through the interior of Market Mall by Fabricland. If you are feeling unwell, i.e. cough, cold like symptoms, please be prepared to put on a new face mask and hand sanitize when entering the facility. You will be greeted by one of our Medical Office Assistants to discuss what brought you in for service. As part of the provincial Connected Care Strategy, the Saskatchewan Health Authority has developed a Community Health Centre at Market Mall which serves older adults (50+) with non-life threatening complex care needs in Saskatoon. The Aim of the Community Health Centre at Market Mall: • The aim of the Community Health Centre at Market Mall is to help you: o Avoid going to hospital unless necessary o Remain at home by connecting with appropriate services o By bringing services closer to home o By providing education on how to stay healthy and safe at home. The Community Health Centre is not a replacement for your family physician. Instead the Community Health Centre team will work with you and your family physician to provide care within your community. We are a new and evolving model of care in Saskatoon. We are always looking for new opportunities and partnerships. As members of the community, you are vital to the development and growth of the Community Health Centre at Market Mall.
